This is a drop leg panel from Eagle Industries. The MOLLE panel allows the user to carry more pouches and accessories (like a medical pouch) on their leg and out of the way. The panel secures to the user with adjustable belt loop and leg straps. There is a mesh pocket on the back of the panel to keep flat items stored, such as maps.

Once again, used appears unissued. I put a general-purpose pouch and two small grenade pouches on mine (not for grenades, just for small stuff), and keep it filled with sundry tools, attached via the belt loops to the headrest of the back seat of my truck. I can be used with a non-molle belt if for some reason I ever wanted to attach it to myself to carry the stuff in it somewhere. You wouldn’t want it on your leg for an extended hike, but for some light walking around it’s more comfortable than I would’ve expected.
